Case Management
Case managers help you to get the care you need. They review your needs and arrange treatment with a health care professional.
Disease Management
These programs focus on long-term problems such as depression and diabetes. They manage care among your doctors, pharmacy, behavioral health professionals and more.
These programs and resources are included in some plans. They are added to others for an additional cost. Some are available only through employers who purchase them for their employees. Find out if you are eligible. Contact your human resources person. Or call the number on your member ID card.
